Additional Documents For Thailand Adoption Application

There are several additional documents which need to be included with the Thai adoption application that are not listed on the application. If you received the Thai adoption application directly from your social worker, she most likely gave you a list of the additional documents. But if you are just beginning this process, here is a list of the additional documents needed:

(1) Police background check from home country of applicant - for US citizens, background checks are provided by the FBI. You may request a copy of your criminal identification record from the FBI.

How to Request a Copy of Record

1. Complete cover letter.
If the request is for a couple, family, etc., all persons must sign cover letter.
Please include your complete mailing address.
If you have a deadline (e.g., an immigration deadline), please include the deadline in your cover letter and on the outside of the envelope.

2. Obtain proof of identity, which consists of a set of your fingerprints (original card, no copies), with your name, date of birth and place of birth. Fingerprints should be placed on a standard fingerprint form (FD-258) commonly used for applicant or law enforcement purposes.
Include rolled impressions of all ten fingerprints and impressions of all ten fingerprints taken simultaneously (these are sometimes referred to as plain or flat impressions.)
If possible have your fingerprints taken by a fingerprinting technician (We went to a local police station and asked if an officer would please fingerprint us. We even used Thai fingerprint cards and then crossed out the Thai and wrote the names; left thumb, etc; in English. We did have a relationship with one of the officers.)
Previously processed fingerprint cards will not be accepted.

3. Include $ 18 - U.S. dollars in the form of a money order, certified check
made payable to the Treasury of the United States, or you may pay bycredit card
Be sure to sign where required
Personal checks and cash are not accepted!
If the request is for a couple, family, etc., include $18 for each person.
If you are paying by credit card you must include the completed credit card payment form
Credit cards will not be used for expedited mail services

4. Mail the items #1, #2, #3 (listed above) to the following address:
FBI CJIS Division – Record Request
1000 Custer Hollow Road
Clarksburg, West Virginia 26306

Please Note: If any of the above items are missing or incomplete, the request will be returned. Allow approximately 16-18 weeks for processing, upon receipt to the FBI.

(2) Home Study - As stated earlier, the home study for the Thailand adoption application will be arranged by your social worker if you reside in Thailand.

(3) Biography for applicant and spouse

(4) Family Plan Document - What type of care will your child receive. We just gave a brief sumary of our family values, methods of discipline and how we show love to our children.

(5) Thai Police Clearance Certificate (similar to the background check done by your home country)- This document is obtained after you have submitted your adoption application to the Child Adoption Center. Your social worker will give you a document to take to the Police Clearance Service Center located at:

Building 24, 1st floor, Royal Thai Police Headquarters, Rama 1, Pathuwan, Bangkok 10330

Phone: 02-205-2168,9

Hours are 8:30 a.m. to 6:30 p.m. Mon. - Fri. 9:00 a.m. to 12:00 p.m. on Sat.

You will need to have a copy of your passport and work permit.

If you are not familiar with Bangkok, the Royal Thai Police Headquarters is located across the street from Siam Paragon and Central World Malls. The best way to get there is to take the Bangkok skytrain to the Chitlom station or Siam station. There is a walkway under the skytrain that connects both stations.

NOTE: Upon entering the maingate of the Police Headquarters, proceed to the guard house and exchange your i.d. for a visitor pass. Building 24 is located to the left of the main mutli-story building in the center of the complex.

When you reach the Service Center, proceed to the information officer. You will be given an application form to fill out. The next step is to take your fringerprints. After washing the ink from your hands, proceed to the photo area. The final step is to meet the investigating officer. The majority of requests for a certificate are for visa applications. You will need to clarify that you need the certificate for an adoption application. This is why the letter from your social worker is so important. The investigating officer will give you a slip of paper to pick up your certificate in 10-14 days. Arrangements can be made to have the certificate mailed to your home address in Thailand.

There may be additional documents required depending on each individual situation.
